My sales teams are taught from the beginning that they must immediately advise the customer when something goes wrong. Explain the steps you are taking to make sure it never happens again, and ask them to tell you what you need to do to make it right.
You may be surprised at how little it really takes to mitigate a problem when you share your genuine concern.
Whatever the situation, just remember you can keep no secrets from your customers.
Because the truth will ultimately reveal itself, so any attempt to hide or delay that revelation only makes matters worse.
